Membership Applications
Note: All membership forms must be signed in the presence of a Candytown Executive Officer (Witnessed) or be notarized. The APPLICANT must print name and sign page 1 of the Membership Application. Additional RIDERS names (immediate family members residing at the same address) must be printed on page 1 of the application. ALL family members intending to enter club grounds must print name and sign page 2 of the application.
In addition to the membership application... Minor riders (under age 18) must submit a Minor Release Form. BOTH parents, and the minor, must sign the Minor Release Form.
